Verified by Psychology TodayWhat Is Kratom Dependence?
Kratom dependence is a pattern of regular kratom use in which the body adapts to the plant's opioid-like compounds and produces withdrawal symptoms when use stops. Kratom (Mitragyna speciosa) is a Southeast Asian tree whose leaves are sold in the United States as powders, capsules, drinks and extracts. Its main compounds, mitragynine and 7-hydroxymitragynine (7-OH), activate mu-opioid receptors, the receptors heroin and oxycodone act on, according to the National Institute on Drug Abuse (NIDA) research summary on kratom, updated in March 2026.
The DSM-5-TR has no separate kratom diagnosis, so clinicians record problematic kratom use as other (or unknown) substance use disorder. The FDA has approved no medical use for kratom, and the FDA announced on July 29, 2025, in "FDA Takes Steps to Restrict 7-OH Opioid Products Threatening American Consumers," that it recommends scheduling concentrated 7-OH products under the Controlled Substances Act. Still Detox lists kratom among the substances that require medical detox at both of its locations.
Why Does Kratom Dependence Need Medical Detox?
Kratom withdrawal follows an opioid-type pattern because kratom's compounds bind to opioid receptors. Singh, Müller and Vicknasingam (2014), in "Kratom (Mitragyna speciosa) dependence, withdrawal symptoms and craving in regular users," published in Drug and Alcohol Dependence, surveyed 293 regular kratom users and recorded the most common kratom withdrawal symptoms: muscle spasms and pain, sleeping difficulty, watery eyes and nose, hot flashes, fever, poor appetite and diarrhea, along with restlessness, tension, anger, sadness and nervousness.
Concentrated 7-OH products raise the stakes. NIDA reports mild to moderate withdrawal for most people who stop regular kratom use, while Stanciu (2026), in a case report published in Cureus, describes a patient who moved from kratom powder to 7-OH tablets and stronger derivatives and then developed fast, severe opioid-type withdrawal. Stanciu and colleagues (2019), in "Kratom Withdrawal: A Systematic Review with Case Series," published in the Journal of Psychoactive Drugs, advised clinicians to treat kratom withdrawal with the same approach used for opioid withdrawal.
Kratom withdrawal treatment at Still Detox starts with specialized 7-OH lab testing, an individualized taper planned around age and medical risk, and comfort care through the acute withdrawal days. Kratom detox follows the same physician-led protocol Still Detox uses for medical detox from alcohol, opioids and benzodiazepines.
Why Choose Still Detox as Your Kratom Detox Center in Boca Raton?
Kratom and 7-OH dependence call for the same medical structure as opioid dependence: physician oversight, round-the-clock monitoring and a direct path into residential care.

My daughter has been in and out of detox, rehab, residential, and PHP for years - with serious substance abuse and mental health conditions. This time around was the first time she made the decision fully on her own, contacted a facility, and was admitted into Still Detox. When I say we've dealt with many facilities, it's a gross understatement. But the team at Still Detox - her therapist Josh specifically - have made an impact on my daughter that we didn't think was possible. Josh has been communicative with me on my daughters progress, and has helped her with grounding techniques for dealing with acute PTSD and dual-diagnosis challenges. She's learning to self-soothe, and for the first time is genuinely putting all of her efforts into taking full advantage of this journey. She has just completed the program, and they assisted in finding a phenomenal facility for her to begin PHP. Just a reminder - no matter how much we love our family members, we can't make the decision for them to change, they have to do it on their own. When the time comes, I strongly encourage you to look into Still Detox as the first step. Sincerely, A grateful Mom![]()

I can’t say enough nice things about this place. The staff here is wonderful; Whitney, Josh, Sam, Kim, Derrick, Elena, Gladys, Dr. Martinez, Mark, et al. When issues come up, as they always will in a rehab situation, things were always handled professionally. The staff here genuinely believes in what they’re doing. The trip down here was totally worth it for the top level of care I received. Would recommend to anyone struggling with an addiction. If you are please reach out to them or someone you trust.
How Long Does It Take to Detox From Kratom?
Detoxing from kratom at Still Detox takes 7 to 10 days on average and extends to 14 days for complex cases, with step-down to residential care around day 8. The Medical Director sets each taper around the client's age, medical risks and recent kratom or 7-OH use.
The table below shows how a typical stay is organized, and exact days vary from client to client. Clients who move to concentrated products face a faster, harder 7-OH withdrawal: Stanciu (2026) reports mild, delayed withdrawal from traditional kratom and rapid-onset withdrawal from potent kratom derivatives.
| Stage | When | What Happens |
|---|---|---|
| Arrival | Day 1 | A nurse and a behavioral health tech meet you together, complete a physical assessment and start specialized 7-OH lab testing. |
| Medical evaluation | Within 24 hours | The Medical Director evaluates you in person. Columbia Protocol risk screening, PTSD and biopsychosocial assessments also take place in the first 24 hours. |
| Supervised taper | Days 1 to 7 | Comfort care and an individualized taper, with nursing staff checking vital signs every 4 hours. |
| Step-down | Around day 8 | You move into residential treatment on the same campus without a new intake. |
| Residential care | Up to 30 days | Daily group therapy, at least one individual session a week and weekly family therapy when family can take part. |
Discharge planning starts at admission, so referrals to partial hospitalization (PHP) and other next steps are in place before residential care ends.
Does Still Detox Offer 7-OH Addiction Treatment?
Yes. Still Detox provides 7-OH addiction treatment and kratom detox for every common product form. Each taper reflects the product, the amount used and the length of use.
Kratom Leaf Products Plant-based
Traditional kratom products deliver mostly mitragynine, which the body converts into 7-OH after swallowing. NIDA reports mild to moderate withdrawal for most regular users who stop.
Concentrated 7-OH Products High potency
Tablets, gummies and shots concentrate 7-hydroxymitragynine (7-OH), a compound the FDA said in July 2025 “can be more potent than morphine.” The FDA reported in July 2025 that these products are sold online and in gas stations, corner stores and vape shops.
How Does Kratom Addiction Treatment Work at Still Detox?
Kratom addiction treatment at Still Detox runs in 6 steps, from a confidential first call through medical detox and into residential care on the same Boca Raton campus.
Confidential Call and Insurance Verification
An admissions representative asks about your kratom or 7-OH use, other substances, medications and health conditions, and verifies your insurance benefits before you commit. Travel is coordinated after clinical approval.
Pre-Admission Assessment
A pre-assessment call within 3 days of your admission date reviews your substance use history, medications and medical conditions, so the clinical team is ready for you on day one.
Arrival and 7-OH Testing
A nurse and a behavioral health tech meet you together for a physical assessment, and the clinical team orders specialized 7-OH lab testing.
Medical Director Evaluation
Within 24 hours, the Medical Director evaluates you in person and sets an individualized taper based on your age, medical risks and recent kratom or 7-OH use.
Kratom Withdrawal Treatment and Comfort Care
Comfort care and medication management ease withdrawal while nursing staff check vital signs every 4 hours. Detox averages 7 to 10 days and extends to 14 days for complex cases.
Step Into Residential Treatment
Around day 8, you step into residential treatment on the same campus for up to 30 days of daily group therapy, individual therapy and family therapy.
What Are the Signs of Kratom and 7-OH Dependence?
Clinicians assess problematic kratom use with the same 11 substance use disorder criteria the DSM-5-TR applies to other substances. Two or more criteria within 12 months indicate a use disorder: 2 to 3 is mild, 4 to 5 is moderate and 6 or more is severe.
Using More Than Intended
Kratom use runs longer or larger than planned. A few capsules a day turns into several doses, and the amount needed to feel normal keeps rising.
Failed Attempts to Cut Down
Repeated attempts to cut back or stop end early because muscle pain, sleeplessness and cravings return within days of the last dose.
Cravings
Strong urges to take kratom or 7-OH push out other thoughts, and cravings drive dose timing through the day.
Time Spent on Kratom
Buying, using and recovering from kratom takes up a large part of the day, including repeated trips to buy more product.
Tolerance and Stronger Products
The same dose stops working, so use shifts from leaf powder to extracts or 7-OH tablets, which concentrate the compound the FDA compares to morphine.
Withdrawal Between Doses
Missing a dose brings muscle aches, watery eyes and nose, hot flashes, diarrhea, restlessness and irritability, the withdrawal pattern documented in regular kratom users.
Problems at Work or Home
Kratom use continues even as kratom causes missed obligations, money strain or conflict with family, partners and coworkers.
Giving Up Important Activities
Hobbies, exercise and time with family fall away as kratom use takes priority, and plans get built around the next dose.

How Do You Detox From Kratom Safely?
To detox from kratom safely, stop under medical supervision with a physician-set taper instead of quitting alone, because 3 risks make unsupervised kratom withdrawal harder and more dangerous.
Withdrawal Drives a Return to Use
Muscle pain, sleeplessness, diarrhea and strong cravings make the first days off kratom hard, and taking kratom again ends those symptoms. Supervised detox treats the symptoms directly, so the urge to redose does not decide the outcome. That is the core of quitting kratom safely.
7-OH Slows Breathing
In laboratory models, 7-OH causes respiratory depression that naloxone reverses, according to NIDA. NIDA also reports that nearly all deaths linked to kratom products involved other drugs or contaminants, which makes mixing kratom with alcohol, opioids or sedatives especially dangerous.
Liver and Other Medical Problems
Case reports link regular, long-term use of large amounts of kratom to serious liver problems, and clinicians report confusion, tremors and seizures in some patients (NIDA). A detox on a hospital campus keeps emergency care close if a complication appears.
